The assassination attempt on former President Donald Trump was the Secret Service’s “most significant operational failure” in decades, agency director Kimberly Cheatle testified on Monday, as she and the agency face increased scrutiny over security lapses during the incident.Key Facts

Cheatle, who testified before the House Oversight Committee, said she would “accept responsibility for this tragedy” during her opening remarks, adding the Secret Service would “look into how this happened, and we are going to take corrective action to ensure it never happens again.”

The Secret Service is investigating why the agency did not place an agent on the roof of the building used by the gunman during the shooting, according to Cheatle, who noted the agency was “still looking into the advance process and the decisions that were made.
