A Republican Plan for Medicare Gets a Revival

NewsHubA number of Republican health care policy proposals that seemed out of favor in the Obama era are now being given new life. One of these involves Medicare , the government health insurance program primarily for older Americans, and is known as premium support.
Right now, the federal government subsidizes Medicare premiums — those of the traditional program, as well as private plan alternatives that participate in Medicare Advantage. The subsidies are established so that they grow at the rate of overall per enrollee Medicare spending. No matter what Medicare costs, older Americans can be sure that the government will cover a certain percentage of it. That’s the main thing that panics fiscal conservatives, because that costs the government more each year.
Premium support could quiet that fear. Subsidies would be calculated so they don’t grow as quickly, thus protecting the federal government (that is, taxpayers) from runaway spending. There are lots of variants, but there are really two principal ideas.
The first is to set the subsidy to a level established by the market , as opposed to one established by the government, as it is now.
One way to do that is to tie the subsidy to the average premium of all Medicare plans, including that of traditional Medicare. This is how the Medicare drug program, Part D , already works. For Part D, Medicare collects bids from all plans that reflect their costs of providing the required, minimum level of drug coverage. Then it sets the subsidy at 74.5 percent of the average bid.
Beneficiaries pay the difference, which will be higher for more costly plans that may offer more generous benefits, and lower for cheaper plans. The system also includes additional subsidies for low-income beneficiaries.
The thinking is that the market drives the subsidy. Because insurance companies want to attract more enrollees, they are motivated to drive their bids downward, driving subsidies downward as well and saving taxpayers money.
If this sounds somewhat similar to how the subsidies for the Affordable Care Act marketplace plans work, it’s because it is similar. Obamacare ties the premium subsidy to the second-lowest premium instead of the average. If an enrollee wants a plan with more benefits but at a higher premium, he or she would pay the difference, not the government.
But even though the approach is similar to Part D — which was passed by a Republican Congress and signed into law by a Republican president — and the A. C. A. marketplaces — established with only Democratic support — it does not have bipartisan endorsement .
That’s just one more example of how congressional actions and attitudes on health care reform are inconsistent. Republicans think subsidies based on bids is an excellent way to reform Medicare, but they don’t laud the Affordable Care Act for adopting the same approach. When it comes to the A. C. A., of course, Democrats supported this mechanism, but they’ve opposed it when it comes to Medicare reform.
Obamacare’s creation of the insurance exchanges and subsidies to expand coverage was a move leftward, supported by Democrats and opposed by Republicans. Anything that relies more heavily on private Medicare options would be a move rightward, and it would probably be opposed by Democrats and supported by Republicans. Such is Washington.
It’s worth noting that progressives are also concerned that this plan might erode traditional Medicare. It could do that because, for a variety of reasons, private plans are likely to bid lower than traditional Medicare. If people have to pay more for traditional Medicare, relative to private plans, they’re likely to leave it, weakening that arm of the program.
The second main idea included in some premium support plans is to further protect the government from rapidly growing expenditures by explicitly capping the growth in subsidies. This could be layered on top of the bidding approach. It would work like this: Plans bid, and the government picks the average or second lowest. Then the government makes sure it doesn’t pay a predetermined amount more than last year — a growth cap.
This kind of cap on subsidy growth is an even more contentious issue. As anyone who follows health care spending knows, it has grown significantly faster than inflation for the past several decades. Putting a more restrictive cap on growth will make budget projections look better. The problem is that such action assumes that there are ways we haven’t previously figured out to reduce Medicare spending without reducing benefits, reducing reimbursement or increasing cost-sharing.
Progressives fear that, given our inability to control health care spending in other ways, this would most likely wind up transferring more and more of the cost of health care onto older Americans themselves. Many would be unable to afford care. The same problems we’re seeing with underinsurance and cost-related access barriers in the private insurance market could become more prevalent.
The entire point of premium support is to rely on the market to innovate and come up with more efficient ways of providing health care and health insurance for it. As such, one cannot say in advance how it would keep costs below a growth cap.
Some people deride this as “market magic,” and it’s easy to see where they’re coming from. It’s not crazy, however, to think that care could be better managed to produce good outcomes more efficiently, at least to some extent. This, in fact, is the theory underlying some of the Affordable Care Act’s reforms, like accountable care organizations.
But the bottom line is this: With premium support, no one can be certain how things will work out. As we consider any premium support approach, we will need to acknowledge that one of the easiest ways to cut premiums is to shift more health care costs to older Americans.

‘NBI man’ linked to Jee Ick-joo case is an errand boy

No officials or agents of the National Bureau of Investigation (NBI) are involved in the abduction and killing of South Korean businessman Jee Ick-joo.
NBI Director Dante Gierran issued this statement a day after President Rodrigo Duterte gave him 48 hours to produce the NBI assets said to be involved in Jee’s case.
“This is a different NBI,” Gierran said at a press conference.
Jee was abducted last Oct. 18, 2016 and killed inside the Philippine National Police (PNP) headquarters in Camp Crame the same day.
The police earlier said that there are also people from the NBI who were involved in Jee’s abduction–a certain “Jerry” who is a driver of a certain “Lising.”
The earlier reported driver Jerry is Jerry Omlang. He sought the NBI’s protective custody over the weekend.
Gierran clarified that he is not an employee or an asset of the NBI.
At the press conference, Atty. Danilo Lising, a supervising agent of the NBI-NCR, denied information that Jerry was his driver.
“I am a mere government employee. I cannot afford a driver,” Lising said.
Lising said he met Omlang when he was assigned at the NBI’s Anti-Human Trafficking Division.
He said Omlang was a “striker” or an errand boy for NBI agents.
When asked if he will resign, Gierran said: “Bakit ako magre-resign, wala naman akong ginawang katarantaduhan (Why should I resign, I did nothing wrong).”
Meanwhile, Justice Secretary Vitaliano Aguirre II urged the public to come forward if they know any information if the NBI was involved in Jee’s abduction and killing.

Cop in kidnap-slay of Korean in PNP custody

Supt. Rafael Dumlao III, the alleged mastermind of the kidnapping for ransom and murder of South Korean businessman Jee Ick-joo, is back in police custody.
Director General Ronald dela Rosa, chief of the Philippine National Police, told a news conference on Monday that Dumlao was in the restrictive custody of the Headquarters Support Service (HSS).
Dela Rosa said he relayed the information on Sunday night to President Duterte, who offered a reward for the capture “dead or alive” of Dumlao, who left PNP headquarters after he was ordered arrested.
“Dumlao, you son of a bitch, if you don’t surface, I will offer a reward of P5 million for you. I am giving you exactly 24 hours. If you don’t come out, that reward goes into effect,” Mr. Duterte said during a news conference in Malacañang late on Sunday.
“And I said dead or alive. If you bring him dead, the better. I do not have to ask questions. I think we already know more or less what happened,” he said, referring to the killing of Jee inside PNP headquarters on
Oct. 18 last year.
About 20 minutes after Mr. Duterte started ranting, Dela Rosa was heard telling him that Dumlao was already in police custody.
Reporters covered their mouths to suppress laughter when it became clear that Mr. Duterte had fallen for a bum steer.
Dumlao, a lawyer, was tagged by SPO3 Ricky Sta. Isabel as the mastermind of Jee’s murder.
Sta. Isabel is the primary suspect behind the abduction and murder of the South Korean businessman.
On Friday night, Dumlao disputed an arrest warrant served by police authorities on him in Camp Crame.
He left the camp and proceeded to his house in Antipolo City on Saturday and refused to heed the warrant when it was served on him again.
Dumlao said the warrant was defective because it merely mentioned “alyas Dumlao” as the subject.
Dela Rosa said Dumlao returned to PNP headquarters on Sunday and he advised him to stop arguing as a lawyer and instead face his accusers.
He also said Dumlao, whose wife is an agent of the National Bureau of Investigation, promised to cooperate in the investigation.
“He is willing to tell all, to give the whole picture of the story according to his version,” Dela Rosa said.
So far, four policemen and four NBI agents have been implicated in the case, he said.
Dela Rosa hinted that some higher NBI officials may be involved in the case.
In his news conference, Mr. Duterte said he had extended his war on drugs up to the last day of his term. —WITH A REPORT FROM AFP

Trump Tells South Korea That Alliance With U. S. Is ‘Ironclad’

NewsHubSEOUL, South Korea — President Trump assured South Korea’s acting president on Monday of the United States’ “ironclad” commitment to defend the country, agreeing with Seoul to strengthen joint defense capabilities against North Korea.
Mr. Trump’s assurances came amid anxiety in South Korea over the future of the alliance with the United States. During his campaign, Mr. Trump cast some doubt on the United States’ defense and trade commitments, saying that South Korea was not paying enough to help keep 28,500 American troops in the country.
But speaking by phone to Hwang Kyo-ahn, the acting president of South Korea, Mr. Trump said that the coming visit to South Korea by Defense Secretary Jim Mattis reflected the close friendship of the two countries and the importance of their alliance. Mr. Mattis is scheduled to visit South Korea on Thursday on his first official trip abroad, which also includes a stop in Japan.
“President Trump reiterated our ironclad commitment to defend the R. O. K., including through the provision of extended deterrence, using the full range of military capabilities,” the White House said in a statement after Mr. Trump’s phone conversation with Mr. Hwang, using the initials for South Korea’s official name, the Republic of Korea.
“The two leaders agreed to take steps to strengthen joint defense capabilities to defend against the North Korean threat.”
Mr. Hwang’s office quoted Mr. Trump as saying that the United States would cooperate with South Korea “100 percent” and that bilateral relations would be “better than ever before.”
The two leaders shared the need for firm action to persuade North Korea to rethink its nuclear weapons development program and vowed to respond strongly if the North attempted military provocations, it said.
Mr. Trump’s call with Mr. Hwang followed North Korea’s recent warnings that it could conduct its first test of an intercontinental ballistic missile “anytime and anywhere,” in a rebuke to Mr. Trump.
Although North Korea has vowed to develop the ability to attack the United States with nuclear warheads and has tested missiles that can cover the Korean Peninsula and its vicinity, it has never tested a long-range missile that could fly over the Pacific.
In a New Year’s Day speech, Kim Jong-un, North Korea’s leader, said his country had reached a “final stage” in preparing to test an intercontinental ballistic missile. The next day Mr. Trump said in a Twitter post, “It won’t happen!”
It remains unclear how close North Korea is to building a reliable intercontinental ballistic missile, although North Korea has boasted of successfully testing crucial technology in the past year, such as long-range engines and heat shields for such a missile.
Mr. Hwang, the prime minister of South Korea, is serving as acting president because President Park Geun-hye’s powers were suspended after the country’s Parliament voted to impeach her in December on charges of corruption and abuse of power.
The Constitutional Court of South Korea is expected to decide in the coming weeks whether to formally end her presidency or reinstate her.

«До сих пор неясно, пойдет ли Украина в направлении, которое для Польши является неприемлемым, то есть поддержки своей исторической легитимности на традиции УПА, традиции организации, которая совершала ужасные преступления против поляков, или же откажется от этого пути», — сказал Ярослав Качиньский в эфире одной из польских радиостанций. По словам политика, сейчас будущее польско-украинских отношений находится под вопросом.
Ранее представители польских властей неоднократно заявляли о негативном отношении к официальным украинским мероприятиям в память об ОУН-УПА и ее активных деятелях.
Обе палаты парламента Польши приняли постановление относительно «Волынской резни», устанавливая 11 июля Национальным днем памяти жертв геноцида, совершенного украинскими националистами против граждан Республики Польша. Фактически, это призыв признать преступления УПА на Волыни в 1943 г. геноцидом. В Украине с этим категорически не согласны, обвиняя польских консервативных политиков в ревизии истории.

What can — or should — tech do about Trump's travel ban?

NewsHubThe American tech community faces a unique hit from Donald Trump’s travel ban against seven countries, but Silicon Valley companies have varied with their reaction to the executive order on border controls.
Over the weekend, the American President rolled out a 90-day travel ban against people from Iraq, Syria, Iran, Libya, Somalia, Sudan and Yemen, wreaking havoc at airports across the country.
The sudden ban caught out tech workers who had been travelling abroad, with an Iraqi-Canadian Facebook worker forced to dash for the border back to Seattle after visiting family in Vancouver and Google recalling staff from overseas to ensure they could come back.
Indeed, staffing – rather than the morality or legality of the ban – was the main public complaint from tech firms, with many pointing out that Steve Jobs was the son of a Syrian. Apple CEO Tim Cook said : «As I’ve said many times, diversity makes our team stronger. »
Google CEO Sundar Pichai said in a statement: «We’re upset about the impact of this order and any proposals that could impose restrictions on Googlers and their families, or that could create barriers to bringing great talent to the US. »
Box CEO Aaron Levie noted that tech firms already face a talent shortage. «Any policy that hurts high-skilled immigration is a disaster. »
More than a staffing issue
Critics noted that the opposition to the executive order often appeared based in financial matters and employment status, with others pointing out that Silicon Valley has been slow to speak out against Trump’s policies before they were put into action.
Some tech leaders did attack the ethics of the executive order, rather than solely the employment impact. Reed Hastings, CEO of Netflix not only criticised the impact on the company’s employees around the world, but said the move was «so un-American it pains us all».
Others highlighted their own experiences and families. «My grandfather came from Poland between the wars, at 17, sponsored by an elder sister,» noted Slack founder Stewart Butterfield. «Two more siblings made it. Everyone else died. »
Facebook CEO and founder Mark Zuckerberg shared similar sentiments. «My great grandparents came from Germany, Austria and Poland. Priscilla’s parents were refugees from China and Vietnam,» he said in a Facebook post, referencing his wife’s family as well as his own. «The United States is a nation of immigrants, and we should be proud of that. » However, that was tempered with praise for a statement that Trump made that the US benefits from «people of great talent coming into the country. »
Satya Nadell, CEO of Microsoft, added: “As an immigrant and as a CEO, I’ve both experienced and seen the positive impact that immigration has on our company, for the country, and for the world. We will continue to advocate on this important topic. »
Such advocacy promises weren’t the only promises of more practical action, with Airbnb co-founder Brian Chesky offering free accommodation to those stuck away from their homes in the US and Google kickstarting a «crisis fund» for the ACLU and other organisations with $2 million alongside donations from others across the industry, and co-founder Sergey Brin making an appearance at protests at San Francisco’s International Airport.
Thiel’s elephant
The elephant in the room with tech firms is Peter Thiel. The venture capitalist is a major Trump supporter, but also sits on the board of Facebook and is an advisor at Ycombinator. Despite Zuckerberg’s personal protest, The Intercept said Facebook had no comment on Thiel’s position at the company, though it’s likely any move to oust the contrarian for his views — controversial as they may be in Silicon Valley — would see equal criticism on grounds of free speech.
Indeed, Thiel has previously suggested it’s hard to speak out in Silicon Valley if your views don’t match the prevailing ones, normally liberal or libertarian.
Back in October, Thiel said Trump’s success «has certainly been hard to accept for Silicon Valley, where many people have learned to keep quiet if they dissent from the coastal bubble.”
Since the election, Silicon Valley has been criticised for just that — keeping quiet against the new powers in Washington, despite louder opposition before the election. Perhaps the threat of a talent shortage will be enough to spark the industry out of that quietude.
Lyft vs Uber
The travel ban was particularly heated between the two leading taxi apps, Lyft and Uber. The latter removed surge charging to JFK amid protests at the New York airport — and as local taxi drivers refused fares in protest of the ban. Some saw that as Uber trying to break the mini-strike, though the company has since stressed it was only trying to help people, including protesters, get to the airport. However, CEO Travis Kalanick has been criticised for working with Trump, and the move sparked a #DeleteUber hashtag on Twitter.
In apparent response, rival Lyft issued a statement against the ban and pledged to donate $1 million to civil rights organisation the ACLU over four years. Uber has since set up a $3 million fund to help its own drivers affected by the order, helping to fund lawyers, compensated them for lost earnings, and offer translation services where needed.
«At Uber we’ve always believed in standing up for what’s right,» Kalanick noted in a Facebook post. «Today we need your help supporting drivers who may be impacted by the President’s unjust immigration ban. »
Kalanick also promised to raise the issue with Trump at the first meeting of the business advisory council the Uber CEO was named to by the president. SpaceX and Tesla founder Elon Musk is also part of that council, which meets on Friday. He suggested in a tweet that the blanket entry ban was «not the best way to address the country’s challenges».
For the sake of Silicon Valley’s tech talent, as well as the drivers at the front line of Uber and Lyft’s financial success, hopefully Musk will bring a stronger argument to the table with Trump on Friday.

В то же время, полицейские пока не называют точное число убитых и раненых. Также, не комментируют, сколько человек задержано.
Исламский культурный центр Квебека на странице в Facebook разместил видео с места стрельбы, добавив подпись «стрельба в мечети Квебека».
Напомним, сообщалось, что во время вечерней молитвы в мечети в Квебеке неизвестные открыли стрельбу. По данным СМИ, минимум пять человек погибли и десятки ранены.

Working with Java Linked List — Developer.com

NewsHubIn Java , LinkedList is a generic class that extends the AbstractSequentialList and implements List , Queue , and Deque interfaces. It is a part of the Java Collection API Library. It basically is an implementation of a type of linked list data structure that facilitates the storage of elements. The contents of this storage can grow and shrink at run time as per the requirement. Quite visibly, it is not very different from the other List classes, such as ArrayList. But, the point of difference is in how the list is maintained at the core. The article is an attempt to delineate the concept behind the data structure involved and its implementation in Java in a terse manner.

A one-dimensional array implies that the elements are stored in a consecutive memory location, organized as a linear list where the size must be defined at the outset of its creation. An array of size, say N, is accessed by its index position through 0 to N-1. Therefore, the first element is represented Array[0], while the last element is Array[N-1].
Now, imagine each array location as a separate «node. » The nodes must be linked in a manner that the next node is accessible from its current node while traversing.
Note that, unlike arrays, the nodes here are not in consecutive memory locations. As a result, there is no way to traverse them consecutively by their indices. Each node must have one or more exclusive parameter that links to the next/previous node.
According to these links arranged, the linked list data structure can be of following types.

A singly linked list arranges nodes in a linear fashion where each node is connected to its immediate next node. The last node links to null , which means end of list. Only one-way traversal is possible in this type of list.

The list is made circular by making the last node point to the first node. The disadvantage of a singly linked list is somewhat overcome here because, once the last node is encountered, the link points again to the first node. The problem of getting lost after reaching the last node in the singly list has now a meaningful pointer whereby it can traverse again from the beginning.

A doubly linked list is yet better because each node links to its immediate previous node as well as to the next node. This makes to and fro traversal from any node except the first and last node possible.

Doubly circular is a modification of a doubly linked list by making the last node again link to the first node and, in a similar manner, the first node again linked to the last node. This makes both clockwise and anti-clockwise traversal possible and is the most flexible of all linked list types.
As should be obvious, the implementation becomes increasingly complex from a singly to circular-doubly linked list. So, let’s implement the simplest one (singly linked list) and the most complex one (circular-doubly linked list) in an old-fashioned way without using the LinkedList class from the Java API library. Though it’s a simplistic implementation, this basic idea can be improved further with numerous clever assumptions.
Here, we create a generic node type class, called SinglyNode. This class contains an information part, called info , and a link part, called next. The information part can be any type of object element whereas the link part must be of its own type. The link part either keeps the connection with another node of the same type or null , if it has nothing to link to.
The use of the Comparable interface is the simplistic example. The following code is superfluous but can effectively used with further assumption and improvement.
The SingleLinkedList class contains the logic for adding and removing elements to the list. The SinglyNode type root always holds the beginning of the chain so that we can begin from there. Care should be taken to never lose the beginning of the list. The next code simplifies adding and removing elements from the list in any location by assuming their indices are like an array, though actually there no such concept of indices in the linked list data structure. It is used to denote the location beginning, end, or middle of the linked list.
This is a circular-doubly implementation. The only difference in the DoublyNode with respect to SinglyNode is the two link pointers, prev and next. The prev links to the node before it and next to the subsequent node. Because it is implemented as circular, the next location of the last node points again to the first node and the prev location of the first node points to the last node.
Observe in the code that the logic for adding and removing elements by their indices is not very different from its SinglyLinkedList implementation, though apparently it may seem complex.
The LinkedList class in the Java Collection API library is a doubly linked list implementation of the List and Deque interfaces that form a generic data structure. The LinkedList object allows null to be one of the elements of the list along with its support of all optional list operations. The class provides a method that can be used to traverse the list from beginning to the end by specifying their indices, similar to array access operations. The best part of using this class that it makes using the linked data structure easy without having to worry about the intricacies involved in implementing one from scratch. However, the LinkedList class is nor synchronized. This means, multiple thread access to it must be synchronized explicitly. The simplest way to achieve this is as follows.
There are many convenient methods in LinkedList such as adding or removing elements to the linked list with the help of a index number, transforming the link list to an array implementation, and so forth. A quick example is given below:
Apart from its internal representation, the working of a linked list is not much different from the other list classes. The power of the linked list lies is its core representation. Though inserting elements at the beginning and end of the list offers a significant performance improvement, the disadvantage of slow insertion and deletion at the middle of the list outweighs them. It is particularly advantageous to use it in a situation where there is a voluminous addition and deletion of elements at the beginning and at the end of the list. In essence, it is no better than ArrayList and should be used only in specific circumstances; otherwise, ArrayList is better.

Union Bank profit breaches P10B

NewsHubAboitiz-led Union Bank of the Philippines chalked up a record-high net profit breaching the P10-billion mark in 2016 on the back of higher interest earnings, fee-based income and gains from sale of securities.
In a press statement on Monday, Union Bank – the first among the country’s big banks to report last year’s results – said net profit surged by 67 percent year-on-year to P10.1 billion.
The earnings performance translated to a return on equity of 16.9 percent.
“We are very pleased with our collective results. All of our major business segments contributed significantly to the bottom-line. More importantly, we were able to scale up our recurring revenue base, while making substantial headway in our journey towards digital transformation,” said Edwin Bautista, UnionBank president and chief operating officer.
The bank’s loan book went up by 31 percent to P235.4 billion last year, allowing total assets to close the year at P524.4 billion.
On the funding side, total deposits grew by 21 percent to P376.5 billion, with low-cost current account-savings account deposits increasing by 18 percent.
The combined growth in customer loans and deposits boosted recurring revenues by 21 percent
to P19.2 billion.
Net interest income expanded by 22 percent to P14.8 billion, while fees grew by 19 percent to P4.4 billion.
The bank reported that full year performance was also enhanced by tax-exempt profits from securities sold in the second half of the year to support the bank’s loan growth strategy. This meant that Union Bank pared down its investments in securities to channel more resources for lending.
